Below is a list of the required forms for the 2023-2024 school year. Returning these forms in a timely manner will assist us in updating our records. Information provided on these forms may be utilized in the event of an emergency. Please read carefully and provide all information legibly. We ask that all of these forms remain attached together, we will separate them when they are returned.

  • Signature Card 

  • Student Contact Information Sheet (Please verify information, provide corrections if necessary)

  • Chrome Book Policy and Use Agreement 

  • Health Survey Secondary Emergency Information

  • Limerick Protective Response Plan

  • Authorization for Student Pick-Up

  • Parental Registration Statement (New Students Only)

  • No Child Left Behind Act Form (Classes of 2024 and 2025 Only)

  • Montgomery County Consent for School Counseling (Permission form for all students. If we do not already have this form on file for your student it will be included in this packet.)


Additional Archdiocesan forms requiring your e-signature are on the Power School Portal. Power School forms are not accessible on the phone app; you must complete them using a desktop or laptop.  


All forms are to be completed and returned no later than Friday, September 15, 2023. Paper forms can be returned to your child’s homeroom teacher.


The Pennsylvania Department of Health requires students in the 11th Grade (Class of 2025) to have a physical examination. Please have your doctor complete the form and return to the attention of the school nurse by Friday, September 30, 2023. We can accept a physical dated any time after July 1, 2023. The PIAA sports physical can be accepted in place of the State physical form:


  • Make a separate copy of the PIAA physical form.

  • Attach a current immunization record and mark the envelope Attn: School Nurse. 

  • We cannot access the trainers PIAA form, and the trainer cannot access the nurses’ forms.

Each student wishing to drive to school and park on campus must complete a parking permit form and purchase a parking permit. Review the following parking regulations and complete our parking permit form.

It is important for student’s safety to be dropped off/picked up at the right side of the curb in front of the school.
