Local school districts that provide transportation for our PJP students require our parents or guardians to complete a request for transportation form.

If your child requires transportation by bus, complete the appropriate school district form below.

 

Forms for the 2026-2027 school year are due by Friday, May 15, 2026.

 

Please be prompt returning these forms. The forms will assist the districts in creating more accurate and timely bus schedules for the next school year. If you submit these forms late you may have to wait a few weeks for your student to be added to a route.

Important Notes:

Please read the district instructions carefully, some districts require proof of residency.

If mom and dad reside in separate locations, two separate district forms need to be completed.

The district in which you reside will share the bus schedule with you a week or two before the school year begins. If you do not receive the schedule please contact your local district.
