Please update the nurses’ office of any change in your student’s health including any injuries, medical treatments, new medications, or immunizations. Up to date information is essential in an emergency situation.
Parents and Guardians complete the Health/Emergency Form for their student at the beginning of each school year. The nurses refer to this form every time that your student visits the nurses’ office. This form indicates your permission for students to receive:Tylenol, Ibuprofen (Advil/Motrin), Chloraseptic, Tums, and Benadryl in the event of an allergic reaction.
**Ibuprofen is limited to 2 doses weekly without written physician permission.
School Immunization Requirements
Immunization regulations for school aged children have changed for the 2017-2018 school year.
NEW PA School Immunization Requirements MUST be met by 5th day of school or the student will NOT be allowed to attend school. If you are unsure, please check with your pediatrician.
Children in ALL grades (K-12) need the following immunizations for attendance:

Only medical exemptions for immunizations are allowed. Those students who have a medical exemption signed by a MD or DO will be accepted. Per Archdiocesan rule, religious or moral exemptions are no longer accepted.
If your child has an exemption, he/she could possibly be excluded in the event of an outbreak of a vaccine preventable disease.
PJP has developed a Concussion Policy through the collaboration of athletics, nursing, administration, and guidance. If a student needs to be evaluated for an injury the concussion policy details the process. Physicians complete PJP's concussion form indicating any needed student accommodations. It is essential that together we are supportive in the recovery process for the health and well-being of our students.
